unique constraint failed error when using multiple output sites

      When using the multiple output sites flag from pegasus-plan, the following error is produced when the workflow runs:

      2020-06-05 15:02:48.445 ERROR [root] Unable to tell database: batch entry 0: [SQLITE_CONSTRAINT] Abort due to constraint violation (UNIQUE constraint failed: rc_lfn.lfn)
